1. Report it Promptly
Please fill out this form as soon as possible after an incident. This ensures the details are accurate and we can act on them quickly.
2. Be Factual and Objective
Stick to the facts of what happened. Describe what you saw and heard, but please avoid guessing at causes or blaming individuals.
Good Example: "During the U12's game, a player tripped over a sprinkler head on the east side of the oval and fell, scraping their knee."
Avoid: "The grounds weren't properly maintained and that's why the player got hurt. Someone should have checked the sprinklers."
3. Be Detailed and Specific
The more detail you provide, the better. Please try to include:
Exact Location: e.g., "Oval 2, near the southern goal post," "in the home team change rooms," or "next to the canteen."
Game/Training Details: Mention the team, age group, and if it happened during a game, training, or break.
People Involved: Note the names of any players, coaches, or team officials who were directly involved or witnessed the incident.
What Happens Next?
Once submitted, this report will be confidentially reviewed by the Club Safety Officer or a designated committee member. They may contact you if more information is needed to take the appropriate action.
For any serious injury, stop and get help first!
If an incident involves a head knock, suspected fracture, breathing difficulties, or any other serious medical issue:
Call 000 immediately.
Notify the team's Coach, Team Manager, or a Club Official right away.
The person's well-being is the first priority; the form can be filled out later.
